Instructions: Please fill in every category as fully as possible. Just use your own words ... don't worry about grammar, punctuation, or spelling (except for the names of people, companies, and places). We'll translate the information you provide into a powerful, effective résumé that will command the attention of prospective employers.
Remember, the information you provide will help us to get to know you and this will help us produce a more effective résumé. We may not use all the information on the final résumé, but it's best if you provide as much detail as possible. We'll sort out what belongs on the résumé. If you have an old résumé, you can fax it to us at 1-513-598-9220.
All the information you provide will be held in the strictest of confidence.
Instructions
Do not use the ENTER key to move between fields, use the TAB key instead. ENTER will cause the information sheet to be transmitted. If you accidentally hit ENTER and the page is transmitted, simply click use your browser's BACK button to return to this form and continue filling out the information.
When you enter information in the larger boxes you can use the ENTER key as a return key. The information you type in will automatically wrap and you can type in as much as you want. The box will automatically expand to hold whatever you type.
You can usually type as much as you want in most of the boxes. If you need additional space, enter the information in the large box at the bottom of this page.
Use the mouse to select options from the drop-down boxes. For the boxes that allow multiple selections, hold down your CTRL key and then click the mouse on as many options as required. To "unselect" an option that was selected, click on that option again.
There is a large box at the bottom for you to enter any other information you wish to let us know about. If you have any questions, please give us a call at 1-513-598-9100 or e-mail us.
