Just use your own words ... don't worry about grammar, punctuation, or spelling (except for the names of people, companies, and places). We'll translate the information you provide into a powerful, effective résumé that will command the attention of prospective employers.

All the information you provide will be held in the strictest of confidence.

Instructions

Do not use the ENTER key to move between fields, use the TAB key instead. ENTER will cause the information sheet to be transmitted. If you accidentally hit ENTER and the page is transmitted, simply click use your browser's BACK button to return to this form and continue filling out the information.

When you enter information in the larger boxes you can use the ENTER key as a return key. The information you type in will automatically wrap and you can type in as much as you want. The box will automatically expand to hold whatever you type.

You can usually type as much as you want in most of the boxes. If you need additional space, enter the information in the large box at the bottom of this page.

Use the mouse to select options from the drop-down boxes. For the boxes that allow multiple selections, hold down your CTRL key and then click the mouse on as many options as required. To "unselect" an option that was selected, click on that option again.

There is a large box at the bottom for you to enter any other information you wish to let us know about. If you have any questions, please give us a call at 1-513-598-9100 or e-mail us.

Objective
It is important for us to know what type of job you are seeking. If the "ideal job" were to become available, how would you describe it? Write a brief description of the type of work you are seeking. Be sure to include factors that are important to you (for example, challenge, opportunity to be creative, etc.) This will help us to write your résumé in a way that will be more attractive to employers hiring for the types of positions you describe. (If possible, fax us copies of employment ads that are of interest to you.)

Strengths and Skills
What do you feel are the most important skills you can offer an employer? These can be tangible (e.g., knowing how to operate a forklift), or intangible skills (e.g., effective negotiating skills). For example, list equipment that you know how to set up, operate, or maintain ... problem solving/troubleshooting ... oral/written communication skills ... leadership qualities ... ability to organize, plan, schedule effectively ... data entry, typing, shorthand skills ... time management ... ability to teach and/or motivate others ... etc. (This section is very important. List as many skills/strengths as you can; this will help us show prospective employers how you stand out from the crowd.)

In the following section, indicate your your level of proficiency. Leave blank if the skill does not apply.

Work Experience

Experience List all your work experience (including volunteer work, if you wish). List each position, starting with your present or most recent, and work in reverse chronological order. Give the most emphasis to those jobs that are related to your objective. Briefly describe less relevant positions. In most cases, jobs held more than 10-15 years ago do not have to be described in detail. Listing the employer, your job title, and dates of employment is usually sufficient. If you have concerns about gaps in employment, age discrimination, lack of formal education, or other problems, let us know. There are ways to develop your résumé so that it minimizes "deficiencies" and optimizes your strengths.

City and State
Employment Dates (Month and Year or just Years) to
Type of Firm
Did you:    Hire?     Fire?     Train?
Number of People Supervised?
Job Title
Duties If you help more than one job with the same employer, briefly describe the earliest jobs first, and show progression/promotions into the most recent position. For example, "Started as file clerk, promoted after six months to secretary where duties included ..." or, "Started as shipping clerk, where duties included packing perishable food products and loading trucks. Transferred to receiving area in May 1999 where responsibilities included inventory control and ..."
Did you use computers or other special equipment? Fill in details here.
Achievements Your achievements will make your résumé stand out from the crowd. Most résumés merely list job descriptions. Only a few will show achievements. Think about things you have done "above and beyond" the normal job requirements. An achievement-oriented résumé is essential in conveying your value to prospective employers. Whenever possible, try to quantify your achievements by using dollar figures, percentages, or other statistics. (Examples of achievement: Promoted with first six months. Salesperson of the Week/Month/Year. Top 10% in sales out of 60 reps. Reorganized inventory/ordering system saving company $5,000 in first year. Opened five major corporate accounts in two months. Wrote procedures manual. Helped implement new computer system. Trained new employees.) The idea is to show the prospective employer how valuable you were to your other companies and how much value you could bring to the job.
